Bring your student number or employee identification. A record is created on your first visit and reused on every visit after that.
In an Emergency
This system keeps health records. It is not a channel for urgent care and it is not monitored outside consultation hours.
For anything urgent, proceed directly to the nearest hospital or health facility, or contact emergency services.
What the Record Contains
- Patient details
- Name, date of birth, sex and civil status, together with the campus and college you belong to, and whether you are enrolled as a student or employed by the University.
- Vital signs
- Blood pressure, temperature, pulse rate, respiratory rate, height and weight, taken by the nurse at the start of each consultation. Date of last menstrual period is recorded for female patients.
- Diagnosis and prescription
- Entered by the University Physician. These entries are permanent: a correction is added as a new entry, and the earlier one is kept and marked superseded rather than erased.
- Requested documents
- Where the Physician asks for a laboratory result or other document, the request is listed against that consultation and you upload the file against it from your own account.
What Happens During a Visit

At the counter
Clinic staff look up your record by name or student number. If this is your first visit, your record is created then.

Vital signs
The nurse takes your vital signs. This opens the consultation in your record and timestamps it.

Consultation
The University Physician records the diagnosis, any notes for you, and the prescription, against that same consultation.

Afterwards
Log in to read what was recorded, including earlier consultations, and upload any document the Physician asked for. First-time patients can set up their own login using their student number.
Campuses Covered
A patient keeps one record across the University. Transferring between campuses does not start a new one.

Who Can See Your Record
Your health record is readable by the clinic personnel responsible for your care and by you. It is not shared with faculty, with your college, or with anyone else in the University.
What each member of staff may do is limited by their role. The nurse records vital signs; the University Physician records diagnoses and prescriptions; neither can quietly remove what the other entered.
Every entry and every amendment is logged against the name of the person who made it, with the date and time. Sign-in attempts are rate-limited, and accounts issued by the Medical Unit require a password change on first use.
